The Northern West Virginia Center for Independent, NWVCIL, a disability advocacy and resource center, is seeking a qualified individual to serve as Transition Advocate. This person will be working with individuals with disabilities wishing to transition from restricted settings such as a group or nursing home back to community living. The Transition Advocate would also be working with young adults as they transition from High School to adulthood.
Responsibilities include assisting the transitioning individual to set goals for community living, identify barriers to reaching and maintaining those goals, and to develop strategies for addressing individual and systemic barriers.
A qualified applicant will have at least 2 years of experience working with individuals with disabilities, have a working knowledge of or ability to quickly learn and navigate WV long term systems of care and community supports and services, good written and oral communication skills and have the ability to travel throughout a 13 county service area.
