Frequently Asked Questions About Methadone in Texas

Do I have to stop using opioids before starting methadone?

For methadone and buprenorphine, continued opioid use during induction is common and expected; the medication itself helps stop use. For naltrexone, however, you must complete a 7–14 day opioid-free period first. Your Texas clinician will tailor the induction based on your medication.

Is a full month’s supply of methadone included in the monthly cost?

For methadone, once SAMHSA take-home criteria are met, the monthly OTP fee covers both in-clinic and take-home doses. For buprenorphine, the clinic fee typically covers the 30-day prescription filled at a Texas pharmacy. For long-acting naltrexone (Vivitrol), the monthly dose is administered at the visit and included in that visit fee.

Does Texas Medicaid cover both the medication and the counseling for methadone?

Yes. Texas Medicaid covers the medication (methadone, buprenorphine, or naltrexone), counseling sessions, group therapy, drug screening, and care coordination as an integrated MAT benefit. Peer-recovery support is also reimbursable in Texas under the state plan.

What happens if my Texas provider bills insurance out-of-network?

Are there license caps or certificate-of-need rules for new methadone clinics in Texas?

Texas applies state-specific licensure review for new OTPs; some states also require a certificate-of-need demonstrating unmet demand. Existing buprenorphine prescribers do not face these caps — only methadone OTPs are subject to location-specific licensure review.

What is the shortest effective course of methadone?

Evidence supports a minimum of 6–12 months of continuous MAT for most patients. Courses shorter than 3 months are associated with high relapse and overdose rates. Your Texas clinician will set an individualized duration target after the first month of treatment.

Can I start methadone through telehealth in Texas?

Yes. Buprenorphine induction and follow-up are routinely delivered by telehealth in Texas under permanent federal rules. Naltrexone can be prescribed via telehealth too. Methadone induction requires an in-person OTP visit for the first dose.

Is the first methadone dose stronger or weaker than later doses?

First doses are intentionally cautious — 5–10 mg of buprenorphine or 20–30 mg of methadone — to assess tolerance and avoid over-sedation. Your dose is titrated up over the first 1–2 weeks until cravings and withdrawal are controlled.

Is constipation on methadone a serious problem?

Constipation is one of the most common and persistent methadone side effects. Daily fluids, fiber, stool softeners, and an over-the-counter laxative like polyethylene glycol are first-line. If severe, your Texas clinician may prescribe a peripherally acting opioid-receptor antagonist.

Will CPS or DCF automatically be called if I use methadone during pregnancy in Texas?

Texas has specific protocols: prenatal MAT under medical supervision is typically not grounds for child-welfare action on its own. Hospital social workers may coordinate a Plan of Safe Care as required by CAPTA — this is a supportive plan, not automatic removal.

Can I skip counseling and just take methadone?

Patients prescribed buprenorphine or naltrexone in Texas can technically receive medication without counseling, but outcomes are significantly better with integrated counseling. Methadone patients at OTPs have counseling requirements per federal rule 42 CFR Part 8.

Will I show up on any public registry for taking methadone?
